Here you will find copies of all the forms you need either for tryouts or during the season.
All players trying out for a 13-17's team must follow the following steps:

Complete the Player Profile Form and mail it to the address below along with a $30 tryout fee and a copy of your USAV membership card (see step 2 below):

Register as a USAV member by going to the ODR website (http://www.odrvb.org/juniors.html) and following the directions for either returning USAV members (meaning you were on a juniors team last season) or new members. Please note that you must pay online when you register due to the new USAV policy requiring all players to be USAV members prior to tryouts. This fee only needs to be paid once per year regardless of how many clubs you are trying out for this season. After you register and pay online, please print out 2 copies of your USAV membership card - one to keep and one to send to us. Everyone should select “Undecided” for your club affiliation when registering. After you accept a position on a team, your club director will add you to the club’s list of players online.

Players 12 and under who are interested in the Instructional League do not have to complete any paperwork at this time. You will receive an e-mail when details for the younger kids have been finalized
